As of May 2021, we are offering both virtual and in-person therapy in our Park Slope location at 110 4th Avenue. For virtual appointments, you will receive an invite to a Google Meet appointment with an easily clickable link for a HIPAA-compliant video session.
We are not currently accepting new referrals, but will be in 2022 as Small Steps expands. Appointments are typically in the afternoons and early evenings for children, adolescents, and adults.
We offer a steep sliding scale based on what you/your family is able to afford.
We do not take insurance, but will provide you with receipts that include all of the information necessary to submit claims for out-of-network reimbursement. It is recommended you check with your insurance company for out-of-network reimbursement rates.
The following codes are most typically billed (add -95 to each code for virtual meetings):
90791 - initial intake/consultation appointment
90834 - individual appointment (45 min.)
90847 - family therapy appointment with client present
90846 - family therapy appointment without client present
Billing/provider information is as follows:
Name: Linda Kudla, Psy.D.
NYS License: 023300
Practice EIN: 85-2842304
Practice NPI: 1659984083
Child and adolescent therapy is often preceded with a parent/caregiver intake appointment, which is 60 minutes and a slightly higher rate than individual sessions.
Subsequent therapy usually includes caregiver involvement, which can range from 10-minute check-ins to full session involvement, depending on need.
Adult therapy also includes a 60-minute intake appointment prior to individual sessions.
Goals are discussed in the first few appointments and are collaborative in nature. We are only interested in working on areas that you find to be important to you and/or your child. You are the expert on you/your family, and it is our role to follow your lead.
